How Lean Body Mass Is Calculated

The Boer formula is one of the most accurate methods for estimating LBM. For men: LBM = 0.407 × weight(kg) + 0.267 × height(cm) - 19.2. For women: LBM = 0.252 × weight(kg) + 0.473 × height(cm) - 48.3. The James formula provides an alternative estimate commonly used in clinical settings.

These formulas provide estimates within 2-4 kg of actual LBM for most people. The most accurate methods for measuring LBM include DEXA scans, hydrostatic weighing, and bioelectrical impedance analysis (BIA), but these require specialized equipment.

Why Lean Body Mass Matters

LBM is the primary determinant of your BMR — the calories your body burns at rest. Muscle tissue burns more calories than fat tissue, even when you are not exercising. Increasing your LBM through resistance training raises your resting metabolism, making weight management easier.

Protein Requirements

Daily protein needs are based on LBM, not total body weight. A common recommendation is 1.6-2.2 grams of protein per kilogram of LBM for muscle building. Using your actual LBM ensures you eat enough protein to support muscle growth and recovery.

Medication Dosing

  • Sufficient protein: Consume 1.6-2.2g per kg of LBM daily, distributed across 3-5 meals
  • Calorie surplus: Eat 200-500 calories above maintenance to provide energy for muscle building
  • Consistent training: Train each muscle group 2-3 times per week with adequate recovery between sessions
  • Sleep and recovery: Muscle growth occurs during rest. Aim for 7-9 hours of quality sleep per night

Real-World Example

A 40-year-old man weighing 200 lbs (90.7 kg) at 5'10" (178 cm):

  • Boer formula LBM: 0.407 × 90.7 + 0.267 × 178 - 19.2 = 36.9 + 47.5 - 19.2 = 65.2 kg (143.7 lbs)
  • Estimated body fat: (90.7 - 65.2) / 90.7 × 100 = 28.1%
  • Daily protein target (2.0g per kg LBM): 130g per day
  • BMR contribution from LBM: Approximately 1,640 calories per day

After 6 months of resistance training and proper nutrition, if his LBM increases to 68 kg while weight stays the same, his body fat drops to 25% — a meaningful improvement in body composition.
